Clinical Pharmacist

Main duties of the job

Our priory CARE access model utilises an AI tool to ensure our patients can access us in a variety of ways which allows both the pharmacists and prescription admin teams to manage patients' requests and needs more efficiently and effectively.

The Clinical Pharmacists provide both routine clinics and specialist clinics across their working week, this is supported by a Clinical Pharmacist CPPE trainer lead and two GP partners with an interest in Workforce transformation.

Within their routine clinics, the clinical pharmacy team provide a combination of Routine Medication Reviews, Acute Prescription requests and Urgent Same Day Queries. The aim being that there is limited hidden work and a complete transparency of daily work load shared between the pharmacists.

There are opportunities for diverse PCN audit work which benefits patient care for which there is protected time created to oversee this.

Each clinical pharmacist is aligned to a specific Priory Medical Group Care Home for which they have protected time and regular interaction with, enabling optimisation of medicine management for this Frail cohort. All our pharmacists have been supported in Frailty training.

Each pharmacist at Priory Medical Group is given an opportunity to develop and be trained in a chronic disease area of interest, benefiting both patients and professional development.

Priory Medical Group (PMG) provides a broad range of NHS and private clinical services covering the whole of the City of York and the surrounding area.

All our surgeries offer GP and Nurse led routine and urgent appointments throughout the week.

PMG was founded in 1946 and has since grown to become one of the largest GP Groups in the UK. Despite our size our core values are to maintain the Family Doctor ethos.

PMG is also a Training Practice and has strong links with the Hull York Medical School and Yorkshire and Humber Deanery. Our doctors train and educate many new doctors each year.

Job responsibilities

Working as part of a wider MDT team within Priory Medical Group, the Clinical Pharmacist role includes:

  • Clinical Medication Reviews and patient monitoring
  • Urgent Medication Requests
  • Same day Medication Queries
  • Care homes and/or domiciliary visit to optimise medicine management
  • Trained in providing specialist chronic disease clinics to match skills and professional interests, telephone or face to face clinics
  • Signposting to community pharmacy and referring to GPs or other healthcare professionals where appropriate
  • Participation in PCN Audits
  • Involvement in patient safety tools and process policy development e.g. STOPP/PINCER
  • Supporting our Prescription Admin Team in their role of medicine reconciliation from secondary care, repeat prescribing and eRD
  • Signposting patients to other members of the wider PCN team, including First Contact Mental Health Workers, Social prescribers, First Contact MSK practitioners, Complex Care Home MDT team etc.
  • Implementing changes to medicines that result from MHRA alerts, product withdrawal and other local and national guidance with support from our GP prescribing lead

Person Specification

Qualifications:

Desirable

  • Completion of an undergraduate degree in pharmacy and registration with the General Pharmaceutical Council
  • NB: it is anticipated level of qualification held may vary according to the level of position and the components of the role being carried out, see person specification for details

Experience:

Desirable

  • Minimum of 2 years experience as a pharmacist, demonstrated within a practice portfolio
  • Have experience and an awareness of common acute and long-term conditions that are likely to be seen in general practice
  • May hold or be working towards an independent prescribing qualification
  • Recognises priorities when problem-solving and identifies deviations from normal pattern and is able to refer to seniors or GPs when appropriate
  • Able to follow legal, ethical, professional and organisational policies/procedures and codes of conduct
  • Involves patients in decisions about prescribed medicines and supporting adherence as per NICE guidelines.
