Shared care agreements with private providers

Shared Care Agreements

We receive many shared care requests from a number of different private clinics and it is important we only take over prescribing in general practice where it is safe and appropriate to do so.

Shared Care from a Private Provider can only be considered in the following circumstances:

  • The provider must be registered with CQC
  • The requester must be GMC or NMC registered
  • A comprehensive shared care document must be shared along with the request eg. with detailed blood, blood pressure or weight monitoring instructions
  • The patient must remain supported/ supervised by the private provider for the initiation period and a reasonable time after establishment of their medication (min 6m of therapy)
  • Our clinical team need to agree with the recommendation, as ultimately shared care is a recommendation and we can choose whether to takeover over prescribing or not (taking in to account whether we feel able to prescribe going forward)
