From Monday 1st September all requests (Blood Sciences, Microbiology or Histology) will need to be done on the Zebra printers - one label per container, with no forms (histology still needs Lab1A Card) – the legacy A4 form will be retired once the new system is live.

The new labels will be a single label per sample container.  The labels will contain patient demographics, key test info, and a barcode that ensure the correct order is entered onto the Lab system.

Histology

Histology requesting needs to be done on ICE.  However, the lab also requires a paper slip to accompany the sample.   To facilitate this the lab ask we attach one ICE zebra barcode label to the paper slip and as one to each sample pot – please see below:

Pots are the same, but request form has changed – details to follow.

Cytology

No change to Gateshead form expected

FIT Test Sampling

For labelling of FIT samples, place the barcoded sticker on the FIT pouch and ask the patients to handwrite their name and date of birth of the FIT device itself.
