Did you know: 1. Hospital doctors in A&E, clinics and wards can issue fit notes for work. These should cover the duration of expected illness and absence from work. 2. You should self-certify for the first 7 days of any illness. You do not need a fit note prior to this. 3. We cannot backdate a new sick note to cover a time when you did not consult with us or another NHS team. For example, if you have taken 10 days off sick with flu, but only contacted us on day 10. We can only issue the fit note from that 10th day. 4. Fit notes can also be issued “may be fit for work” if you are well enough to work but need a phased return, reduced hours or adaptions. 5. You can return before your fit note expires if you feel well enough. There is no requirement for a staff member to be “signed fit” to work by a doctor. 6. In 2022 guidance was updated - nurses, occupational therapists, pharmacists and physiotherapists can legally certify and issue fit notes (according to their training and competency). Previously only GPs or hospital doctors could do this.

prioryCARE
The prioryCARE form, uses the Klinik total triage tool which does much more than just allow you to input a request for an appointment. You can also; ask for test results, extend a fit (sick) note, let us know you are a military veteran or send us a query regarding your medication. You can read more about this on our appointments page.
If you have a new or changing medical problem, the system will take you through a series of questions about your symptoms. We aim to action urgent queries within 1 working day and routine queries within 3-4 weeks. This system does not guarantee a face to face appointment, but each new query will be assessed by a clinician and they will act on them depending on the clinical need. This may mean we arrange a telephone consultation, send you a text with advice or book you a face to face consultation at your local surgery.
The NHS App
Owned and run by the NHS, the NHS App is the simple and secure way to access a range of NHS services on your smartphone or tablet. You can request repeat prescriptions, change your nominated pharmacy, view a referral and much more. We recommend you keep notifications on so you can see if a message has been sent. SystmOnline
You can request repeat prescriptions, access some pre-bookable appointments (e.g. BP check, smear test) and review your medical record.
To register:
- Patient Care Navigators (previously known as Receptionists) at all of our surgeries can set you up to access our on-line services. You will need to bring some photographic ID into the surgery (Passport or Driving Licence) to confirm your identity
- They will register you on the system and give you printed confirmation of your login details and instructions on how to use the system
- We would encourage you to change your password the first time you login to the new system to something more memorable for you. All passwords must be a minimum of 8 characters and must contain at least one number and one alphanumeric character (e.g. ‘!’ or ‘?’). Please allow one hour from registration before attempting to logon to our on-line services.

Proxy Access
If you need to request medication or review the medical record for your child, relative or someone you care for, it can feel confusing how to do this.
Adult SystmOnline Access
- If you would like to grant a carer/relative access, both the patient and carer need to be registered with the practice
- We will ask for both to provide ID and the patient to provide a signed letter consenting to the individual having access to theor medical records.
- Our clinical/safeguarding team will review the request and approve if appropriate and safe to do so.
Child SystmOnline Access
- From birth to 11th birthday, parents can apply for full SystmOne online access - complete paperwork, provide ID and birth certificate via the reception teams. Both parent and child need to be registered with a SystmOne practice.
- On the child’s 11th birthday access is turned off, the practice may allow the parent access to medication requests and appointment booking - this will be reviewed by the clinical and safeguarding team to ensure it is appropriate to grand access and that consent has been obtained from the parent and child.
- On the child’s 16th birthday parent online access is removed as the patient is deemed competent to have access to their own records.
Child NHSapp
- A child can sign up for their own NHSapp account at the age of 13.
