8:20am update: the system now appears to be working (a little slower than usual). Please bear with our reception and patient central teams as they catch up. We also have some clinical sickness today and this will mean some postponement of appointments. We are unable to access SystmOne this morning - this is our computer system that holds and documents patient records and appointments. We have logged an urgent report with N3i, the IT provider for the local area. They are working on resolving this ASAP. The prioryCARE form will turn back on once we have access to SystmOne. We understand this may be a York-wide issue.

Patients with a diagnosis of stroke will have an annual check of their blood pressure to ensure it is in a healthy range. They will also have a medication review to ensure they are on the correct medication and that all the necessary annual blood tests are up to date
