Opting out of sharing your confidential patient information

DEADLINE FOR REGISTERING YOUR OPT-OUT IS JUNE 23rd 2021

NHS digital have set a deadline of 23rd June 2021 for patients to prevent their patient data being shared for purposes except their own care, this is known as registering a type 1 opt-out. Patients can still opt out at any time after this date, but your data may have been shared in the interim.

NHS digital will start collecting data on 1 July 2021 so if patients do not want their data to be shared with NHS Digital they need to register a type 1 opt-out with the surgery by 23 June 2021.

The data held in the GP medical records is shared with other healthcare professionals for the purposes of the individuals care. It is also shared with other organisations to support health care planning and research. If patients do not want their data to be shared outside of the practice for purposes except their own care, they can register an opt-out with the practice, known as a type 1 opt-out.

To register a type 1 opt-out, for yourself or a dependent (if you they are the parent or legal guardian of the patient) (to Opt-out) please complete the form below and return it to surgery in person or by scanning and emailing it to voyccg.prioryoptout@nhs.net before the deadline of June 23rd 2021.
